It's worth separating two different questions people often blend together. "Real vs. fake" is about whether nacre is present at all. "Natural vs. cultured" is about how that nacre came to be. A natural pearl forms without any human involvement — genuinely rare today. A cultured pearl forms because a farmer deliberately introduces an irritant into a mollusk and lets nature do the rest over months of careful cultivation. Both are real pearls, built from the same nacre, subject to the same tests above. Caring for Pearls Once You Know They're Real
